Prostate cancer

Ang prostate cancer ay karaniwang tumatama sa mga kalalakihang nasa 60-anyos pataas. Kung hindi pa malala ang prostate cancer, maaari pa itong maagapan. Ngayon ay maaari na ring mamili ang mga cancer patients kung sa anong paraan sila gagamutin – sa pamamagitan ng operasyon (surgery) o sa radiation theraphy.

Kamakaila’y dumalo ako sa taunang meeting ng American Society for Therapeutic Radiology and Oncology na ginanap sa Arizona. Sa meeting na iyon ay natalakay ang mga advance na pamamaraan sa pangangalaga ng may prostate cancer. Ang mga advance na pamamaraang ito ay ginagawa sa mga modernong cancer centers sa United States sa kasalukuyan.

Ang operasyon at ang radiation therapy ang dalawang pamamaraan kung paano iti-treat ang may prostate cancer. Sa dalawang ito, ang radiation therapy ang sinasabing pinaka-epekto sa mga may prostate cancer. Ginagawa ito sa pamamagitan ng brachytherapy (radioactive seed implants) at external beam radiation theraphy. Sinasabing 80 porsiyento ng mga may prostate cancer ang gumaling. Ito ay batay sa limang taong pag-aaral sa mga may prostate cancer. Gayunman may kamahalan ang pamamaraang ito kung ikukumpara sa operasyon.

Gayunman, may mga pasyente na hindi maaaring maisagawa ang brachytheraphy dahil sa mga anatomic reasons o medical conditions. Sa halip isasailalim sila sa external beam radiation therapy. May katagalan nga lamang ang pagsasailalim ng pasyente sa pamamaraang ito. Kailangan ang araw-araw nilang exposure na tatagal sa loob ng anim hanggang pitong linggo kumpara sa brachytheraphy na ginagawa sa loob ng ilang oras.

As far as the treatment of prostate cancer is concerned, the use of androgen (male hormone) suppression in addition to radiation theraphy has significantly improved local control of prostate cancer and reduced the incidence of its metastatic spread. Patients with locally advanced prostate cancer are now given hormonal agents (Flutamide and Lupron) prior to and during radiation theraphy. These agents which deprive the prostate cancer of male hormones produce good local control and significantly reduce metastasis and cancer progression.

The American College of Radiology is now considering giving higher doses of radiation to prostate cancer patients by three dimensional conformal radiation theraphy (3 D-CRT) using computers in the planning process. This allows radiation onocologists to focus the external radiation beam directly to the cancerous area, avoiding damage to sorrounding healthy tissues with a survival rate of 88 percent.

Men who have pre-treatment (PSA) prostatic serum antigen levels of less than 10 (indicating early stage cancer) have a biochemical survival rate of 91 percent for those with pre-treatment PSA levels of more than 10 percent.
