CEBU, Philippines - The Philippine Health Insurance Corporation will be issuing Certificates of Good Payment Standing (CGPS) at the SM Business Service Center and at Nerbac until January 29, 2010.
The PhilHealth desk at the SM Business Service Center is open to serve the public Monday to Friday at 10:00 a.m. to 4:00 p.m. The desk is located at the second level of SM City Cebu (near Prime Care).
Nerbac is open Monday to Friday, 8:00 a.m. to 5:00 p.m., and is located at the LDM Bldg., Lapu-Lapu Street in Cebu City.
The CGPS serves as ‘clearance’ of a business establishment, attesting that it is in good standing with PhilHealth in its remittance and report of personal and/or employee contributions to the social insurance fund. Business establishments need this CGPS to secure a business permit from city hall.
PhilHealth issues this CGPS to employers or proprietors of business establishments who are able to present proof of contribution to PhilHealth of the immediate 12-month period prior to the month the CGPS is requested.
Corporations and partnerships requesting for the CGPS must bring a copy of their SEC registration together with the applicable remittance reports and receipts of payment.
Individually-paying members must bring proof of personal contribution to PhilHealth together with its remittance receipts and report of employee contributions (if any) and its DTI registration.
Cooperatives must bring a copy of their registration documents with the Cooperative Development Authority (CDA) along with its employee remittance reports and payment receipts.
The PhilHealth members assistance center (PMAC) at the ground floor of Golden Peak Hotel and Suites, Gorordo Ave. corner Escario St., Cebu City issues the CGPS throughout the year to establishments applying for business permits for the first time as well as those applying for renewal of their permits.
PhilHealth service offices in Mandaue City, Danao City, Carcar City, Bais City in Negros Oriental, Dumaguete City, Talibon in Bohol, Tagbilaran City, and Siquijor also issue CGPS to establishments in their respective catchments.
PhilHealth Collection Section had earlier been pre-issuing CGPS to establishments recognized to be of good payment standing in anticipation of the bulk of clients visiting PhilHealth offices to secure the same. The CGPS has a validity of three months from date of issue.
